Truck Driver Investigated Over Fatal Train Crash
According to the Los Angeles Times, the driver, who also died in the crash, had five traffic violations since 2008, four of them for speeding.

Tanker Truck Driver Dies in Fiery West Virginia (WV) Crash
While the cause of the West Virginia (WV) crash isn’t known, any single-vehicle crash early in the morning will lead investigators to look at whether driver fatigue was a factor. According to a 2006 Federal Motor Carrier Safety Administration report, about 13 percent of truck accidents are caused by fatigue.
